Consumer Rights

In accordance with House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, Texas-licensed behavioral health providers are required to make the following consumer information publicly available.

This notice is provided in compliance with Texas state licensing requirements and Section 181.105 of the Texas Health and Safety Code.

Consumers have the right to:

  • Request access to their own health care records
    To request health care records, email jeneversdyk@gmail.com. Requests are processed in accordance with applicable state and federal law. Fees may apply as permitted by law.

  • Contact the Texas Behavioral Health Executive Council (BHEC) regarding our work together.
